Clinic Policies

New Patient Registration: New patients are required to arrive 30 minutes prior to their first appointment to allow for time to complete New Patient Registration forms.

Insurance Updates: Patients must provide the clinic with any updates or changes to their insurance policy to ensure accurate billing and coverage.

Late Policy: Patients are allotted a 15-minute grace period. If a patient arrives more than 15 minutes late for any reason, they may be asked to reschedule their visit.

No-Show Policy: While we do not charge late or no-show fees, patients who miss three or more appointments without notice may be discharged from the practice and referred to a different provider for ongoing care.

Medication Refills & Health Questions: Patients may request medication refills, follow-up on health questions, or check orders/referrals via the HPH MyChart portal or by phone.

Please allow for 2-3 business days for your provider to return your refill request, phone call and/or MyChart message.

Your Provider reserves the right to request an in-person or telehealth visit to address any medical questions/concerns or paperwork that cannot be adequately handled remotely.

Zero Tolerance Harassment Policy: We have zero tolerance for any verbal or physical harassment toward staff or providers. Abusive behavior will not be tolerated and patients' who exhibit this behavior may be discharged from the practice.
